We’ve enhanced our ENROLLMENT/ADD/TERMINATION FORM to include additional information on COBRA elections.

• You can direct employees to the writable form to complete, print and sign. (employee & employer signatures required)
• The new form provides a legible print version and will help eliminate errors.

Please note:  We’ve enhanced this section to improve COBRA transactions.  For transactions to reinstate a member to COBRA, please check off TRANSFER TO COBRA, then HNE COBRA.  If your group has a Health Reimbursement Arrangement (HRA) with Health Equity and the member has elected HRA as part of their COBRA election, please check off HNE COBRA WITH HEALTH EQUITY HRA.  By checking off HNE COBRA with Health Equity HRA, it will allow the data feed to continue the integrated eligibility and claims feed to Health Equity.
