The writable New Group Application Form can be found under the Employers and Brokers tabs on the HNE website. The writable Enrollment Form can be found under the Members, Employers, and Brokers tabs.

Employers can direct their employees to complete the Enrollment Form on the HNE website.

Using the writable forms will reduce processing errors that result from deciphering handwriting and reduce ID card reissues due to incorrect spellings and incorrect addresses.
