Who is a Candidate for Integrated Care?

Any HNE member can be a candidate for Integrated Care Management. A member may be identified by:
- Complicated illness or multiple health risk factors
- Physician or provider referral
- Referral following an inpatient hospital stay
- Medical and pharmacy claims
- Member request
An HNE Care Manager will screen referred members to assess medical, behavioral health, and social needs. The resulting care management plan may include:
- Care Coordination
- Disease Management
- Coaching
- Intensive care management
This depends on the specific needs of the member.
The Benefits of IntegratedCare Management
- Help members get the care and services they need
- Identify community resources for members, their families, and caregivers and help with the referral process for these resources
- Educate members, their families, and caregivers about medical, psychological, and substance abuse conditions that affect overall health
- Support and encourage the doctor-patient relationship
- Encourage the member to participate in his/her own care
- Help members follow their medication regimen
- Help manage the costs associated with medications
