Any HNE member can be a candidate for Integrated Care Management. A member may be identified by:

• Complicated illness or multiple health risk factors
• Physician or provider referral
• Referral following an inpatient hospital stay
• Medical and pharmacy claims
• Member request

An HNE Care Manager will screen referred members to assess medical, behavioral health, and social needs. The resulting care management plan may include:

• Care Coordination
• Disease Management
• Coaching
• Intensive care management

This depends on the specific needs of the member.

• Identify community resources for members, their families, and caregivers and help with the referral process for these resources
• Educate members, their families, and caregivers about medical, psychological, and substance abuse conditions that affect overall health
• Support and encourage the doctor-patient relationship

• Help members follow their medication regimen
• Help manage the costs associated with medications
