The Health New England website, www.hne.com, has added the New Group Application Form and Member Enrollment form in Adobe Reader PDF format!

Using writable forms will alleviate processing errors in deciphering handwriting and reduce the need to reissue ID Cards for incorrect spellings and incorrect addresses.
